Form 1095-C

Ohio State provides you with a tax form, called a 1095-C, as part of our reporting requirements under the Affordable Care Act (ACA). The 1095-C contains important information about medical coverage offered to you and your dependents by Ohio State.

Child Care Program

The Ohio State University Child Care Program provides quality developmentally appropriate early education for the children of university students, faculty and staff. Ohio State is proud to offer this program as a department of the Office of Human Resources. As an active and integral program of the university community, the Child Care Program also supports Ohio State’s academic mission through learning opportunities for Ohio State students and faculty. Academic activities may include research, class projects, observations and field placement.
