PHA deadline change. To receive premium credit, complete by December 14, 2020

To reduce your cost for health care and receive the full premium credit for 2021, complete a Personal Health and Well-Being Assessment (PHA) by Monday, Dec. 14, at 11:59 p.m. The PHA will not be available for completion starting December 15 to prepare for the transition from PeopleSoft to Workday. The PHA should be available again in early January.
