Your Plan for Health has a new quarterly incentive structure that gives you the opportunity to earn rewards more frequently throughout the year, including Health Reimbursement Account credits, PulseCash and medical plan premium credit.

Consent needed for electronic delivery of 1095-C tax form

Form 1095-C is a required tax form that provides information about the health care coverage you had or were offered by Ohio State in 2018. If you have not yet consented for electronic delivery, please do so by February 11. Otherwise, the form will be delivered to your home address on file in the HR System by March 4.
