2025 Benefits Reminders

As a reminder, 2025 Open Enrollment elections you made will take effect on January 1, 2025. This notice contains important information to remember as we move into the new year.

Accessing Your Benefit Information

Manage your health benefits through each vendor website and/or mobile app. You can access information such as the status of your deductible and out-of-pocket maximum, view claims and show your ID card to providers. You will only receive a new Medical ID Card if you made a medical plan change during Open Enrollment. If you did not make a medical plan change, continue to use your current card.

Coordination of Benefits

If you have a dependent enrolled in an Ohio State medical plan, you are required each year to complete a Coordination of Benefits (COB) form. In January, Luminare Health will mail letters with instructions on how to submit the required information. Your prompt reply assures no delay in the processing and payment of your medical claims.

Flexible Spending Account Information and Deadlines

Expense and Reimbursement for 2024 Plan Year:

You have until March 15, 2025, to use the FSA funds in your 2024 account on eligible expenses. This date window includes the grace period.

Reimbursement Filing Deadline for 2024 Plan Year:

By March 31, 2025, all requests for reimbursement must be received by HealthEquity or you will forfeit the unclaimed balance in your FSA(s).

Vendor Updates

Ohio State contracts with vendors to deliver our comprehensive suite of benefits. In 2025, there are a few updates with some of our partners:

Form 1095-C

There is a new administrator for your Form 1095-C required by the Affordable Care Act. Trusaic is Ohio State’s new vendor to provide 1095-C forms. To receive an electronic copy of your 2024 1095-C in place of a paper copy, you are required to complete the registration and consent through the Trusaic website by January 15, 2025. Review instructions on receiving an electronic copy.

Employee Assistance Program

Ohio State’s Employee Assistance Program vendor is changing its name from “Impact Solutions” to “AllOne Health.”

Your Plan for Health

Ohio State’s wellness platform “Virgin Pulse” is now “Personify Health.”

Protecting Your Personal Information

Please be cautious that telephone scam and email phishing attempts can increase during this time of year. If you are contacted by someone saying they are an Ohio State vendor, do not provide personal information. Legitimate vendors will not call you to ask for confidential personal information and will never request confidential information through email.
