Benefits Confirmation Statements now available online

Faculty and staff eligible for Ohio State benefits can review their 2019 Benefits Confirmation Statement in Employee Self Service today through November 30, 2018. Employees will also receive a copy of the 2019 Benefits Confirmation Statement at their home addresses.

The statement reflects the enrollment status in university-sponsored medical, dental, vision, flexible spending accounts (FSA), life insurance and short-term disability plans that will be in effect from January 1, 2019, until December 31, 2019.

It is very important that employees review their 2019 benefits elections and verify that all are accurate. This is the final opportunity, outside of a qualifying event, to change your elections.

To view your Confirmation Statement online:

  1. Visit Employee Self Service
  2. Click View Confirmation Statement (below eBenefits)

If no corrections are needed, print the statement for your records.

If the benefit elections are inaccurate, you may make changes using one of the methods below:

Click on Open Enrollment on the Employee Self Service home page. Make your changes and click Authorize and Submit after you have made changes. Print an updated confirmation statement for your records.

OR

Print the confirmation statement (or use the copy mailed to your home address) and write corrections directly on it, then sign and date the statement. Fax the completed confirmation statement to 614-292-7813 or return via mail or drop off to the Office of Human Resources Benefits Services, 1590 North High Street, Suite 300, Columbus, OH 43201.

All changes must be submitted by 11:59 p.m. EST on Friday, November 30, 2018. For benefit plan details, visit hr.osu.edu/oe.

Supplemental retirement enrollment calendars to change April 1

In addition to the mandatory retirement plans, Ohio State offers supplemental savings options to help faculty and staff plan for retirement. Most Ohio State employees are eligible to participate in the 403(b) and 457(b) Supplemental Retirement Accounts (SRA). Due to changes in federal law, Ohio State’s enrollment rules for 457(b) plans will align with those of the 403(b)

Although Ohio State strives to remain open, faculty and staff can be prepared by reviewing this policy and taking note of the resources.

First Quarter Deadline for Lifestyle Spending Account Claims

Ohio State’s Lifestyle Spending Account (LSA) reimburses employees up to $125 quarterly for eligible expenses related to health, fitness, family, emotional and social wellness. The first quarter ends March 31. Employees have until April 15 to submit a claim for eligible expenses incurred January 1 – March 31. Unused LSA funds do not carry over.

Enrollment Window Opens for Summer Semester

Applications for tuition assistance benefits must be submitted each term, and the enrollment window for summer semester 2023 is March 20-May 12, 2023.

Policy Reminder: Nepotism

The Nepotism Policy 1.25 emphasizes that selection of personnel be based on job-related qualifications. Individuals must notify their supervisor if any family member applies for a position for which they will be responsible or may influence the employment actions.
