Benefits confirmation statements now available online

Faculty and staff enrolled in Ohio State benefits can review their 2017 Benefits Confirmation Statement in Employee Self Service today through December 2, 2016.

Enrolled employees will also receive a copy of the 2017 Benefits Confirmation Statement at their home addresses. The statement reflects the enrollment status in university-sponsored medical, dental, vision, flexible spending accounts (FSA), life insurance and short-term disability plans that will be in effect from January 1, 2017, until December 31, 2017.

This is the final opportunity, outside of a qualifying event, to change 2017 benefits elections. It is very important to verify that all elections are accurate.

To view the Confirmation Statement online:

  1. Visit Employee Self Service
  2. Click “View Confirmation Statement” below eBenefits.

If no corrections are needed, print the statement for your records.

If the benefit elections are inaccurate, click on Benefits Enrollment on the Employee Self Service home page. Be sure to click Authorize and Submit after you have made changes. Print an updated confirmation statement for your records.
