Policy Reminder: Self-Disclosure of Criminal Convictions - Human Resources at Ohio State

Policy Reminder: Self-Disclosure of Criminal Convictions

As part of The Ohio State University’s Self-Disclosure of Criminal Convictions Policy, all current faculty, staff, graduate associates and student employees, appointees, volunteers and staff provided by third party vendors are required to self-disclose criminal convictions that occur within three business days of the conviction.

Associated faculty who have a break in service of less than 12 months must disclose any convictions that occurred during the break within three business days of returning to university employment.

Criminal convictions will not necessarily bar continued employment. Once a conviction is disclosed, a determination regarding suitability for continued employment will be made based on the information received and collected regarding the conviction.

Contact Information:

  • For a disclosure form, please contact your college/unit Senior HR Professional or the Office of Human Resources Employee and Labor Relations team at hr-elr@osu.edu or (614) 292-0864
  • For information on the Wexner Medical Center disclosure process, please contact Wexner Medical Center Employee Relations at (614) 293-4988
  • For policy clarification, please contact the OHR Background Check Coordinator at (614) 688-5971 or HR-Backgroundchecks@osu.edu
