Your Plan for Health (YP4H) is our comprehensive wellness program designed for employees and their spouses. It integrates a variety of programs, resources and rewards to help you live healthier, feel better and strengthen your overall well-being.

You have access to a range of options, so you can focus on what matters most — your health and wellness.
YP4H Portal
Starting in 2026, the YP4H portal, powered by MediKeeper, is your hub for accessing a variety of wellness resources offered by Ohio State to support your physical, emotional, social and financial well-being.
The portal brings all your well-being resources into one hub, including access to biometric screenings, nutrition support, fitness programs, health coaching, wellness classes, resources for expecting parents, chronic condition coordination and more.
Sign up and access the portal
- Start Here: Go to yp4h.osu.edu or download the Alaveda app (by MediKeeper, Inc.) from the Apple App Store or Google Play Store. Tip for mobile app users: If you prefer to register for the first time using the Alaveda mobile app, use code “osu” to set up your account.
- Create An Account: Use your legal first name, last name, and date of birth exactly as shown in Workday.
- Choose Your Login: Pick the email address you want as your username. Choose a password you’ll remember.
- Secure Your Account: Set up a method to two-factor authenticate your identity (email, SMS or authenticator app).
- Log In Anytime: Continue accessing the portal through yp4h.osu.edu or via the Alaveda app.
Incentive Program Overview
YP4H features an incentive program designed to make your health journey even more rewarding. Through the YP4H portal, you can earn rewards by participating in a variety of wellness activities.
Earning Points
Between Jan. 5 and Nov. 30, 2026, you can earn points for each eligible activity completed in the portal. As you participate in wellness programs and activities, your points will accumulate throughout the year.
Redeeming Rewards
As your points add up, you have the flexibility to choose when and how you redeem them for rewards. Available options include a Benefit Credit toward your medical premiums, Health Reimbursement Account (HRA) Funds or gift cards. Be sure to redeem your points and select your rewards by Dec. 31, 2026.
Who Can Participate
- Ohio State employees eligible for medical benefits
- Spouses of Ohio State employees who are enrolled in an Ohio State medical plan, once your medical coverage is active
- OSU Physicians and Central Ohio Technical College (COTC) employees enrolled in an Ohio State medical plan
- Graduate students enrolled in an Ohio State employee medical plan
Employees new to Ohio State: If you plan to elect an Ohio State medical plan, wait until your election has been fully processed before registering, earning points and redeeming rewards. This will limit the disruption you may experience accessing your account and ensure the information you view is accurate based on your eligibility to participate in the program.
How to Earn Points
Within the YP4H portal, you can accumulate points by participating in a range of wellness activities. These activities are organized into four main categories: prevention, participation, programs and progress. Each category offers multiple opportunities to earn points and support your overall well-being.
The largest portion of points can be earned by completing preventive care activities. This includes scheduling and attending an annual well visit or physical with your primary care provider or participating in an on-campus biometric screening.
Additional actions such as dental cleanings and vison exams, and recommended screenings for mammography, cervical cancer, colon cancer, prostate cancer, skin cancer and osteoporosis also qualify. Vaccinations, including flu, COVID, shingles, Tdap and pneumonia, are eligible for points as well.
To document completion, you may either upload proof directly to the YP4H portal or opt-in to automatically share your data, which allows the OSU Health Plan to verify completion for you.
Top ways to earn points include:
- Preventive exams and screenings (up to 3,250 points)
- Immunizations and vaccines (up to 750 points)
Health screenings for OSU Health Plan (OSUHP) members
OSU Health Plan (OSUHP) members can choose to automate their health screening rewards. Please review the consent form carefully to decide whether you want to allow OSUHP to process your health care claims for the verified-only rewards. If you prefer not to provide consent, you may manually upload documentation in the portal to earn the same rewards. Once consent is given, OSUHP will begin processing your rewards within 2–3 business days and will retroactively include activities completed since Dec. 1, 2025.
Engage in a variety of challenges and activities regularly offered in the portal to earn points. Examples include completing the Personal Health Assessment (PHA), participating in the Wellness@Work Survey, joining Team Challenges, Pal-to-Pal Challenges and posting in Community Circles.
Top ways to earn points include:
- Personal Health Assessment (PHA): 500 points
- Wellness@Work Survey: 250 points (available only for employees)
- Team and Pal-to-Pal challenges (1,500+ points)
Structured wellness programs available in the portal provide additional opportunities to earn points. These include Health Coaching sessions, LifeSpeak fitness classes, Avidon Wellness Courses and Plans for Wellness, each designed to support your health goals.
Top ways to earn points include:
- Avidon Wellness, courses and health coaching (1,600+ points)
- Lifespeak Holistic Wellness (begins Jan. 21 – 3,000+ points)
- Plans for Wellness
Earn points by tracking your wellness progress. This may involve syncing a fitness tracker or manually logging your stats such as steps, sleep hours and workout minutes. Completing races or participating in organized sports are other ways to accumulate points under this category.
Top ways to earn points include:
- Physical activity and movement tracking (3,400+ points)
Extra points are available when you complete two qualifying activities, based on your eligibility for the program. For instance, enrolled employees can receive bonus points by completing:
- PHA and an annual well-visit or physical (750 points)
- PHA and Wellness@Work Survey (500 points)
Redeeming Your Wellness Points for Rewards
As your points accumulate, you can redeem them for rewards through the YP4H portal. Be sure to redeem your points and select your rewards by Dec. 31, 2026. Any points that remain unused after this date will be forfeited and will not carry over into the following year.
The reward options available to you depend on your enrollment status in an Ohio State medical plan and shown in the YP4H portal at the time you redeem your points. To view your current choices, log in to the YP4H portal and access the “Incentives & Rewards” section.
To redeem your rewards, navigate to the YP4H portal and click on the “Redeem My Points” tile. This will allow you to redeem points for the rewards you are eligible for.

For 2026, you can redeem a maximum of 8,000 points – 3,000 can be redeemed for your benefit credit and 5,000 points that can add up to $500 HRA credits. You choose when and in which order you want to redeem your rewards.

This year, you can redeem a maximum of 5,000 points that add up to $100 in Rewards Bucks. You choose when and in which order you want to redeem your rewards.
