If you wish to enroll in Ohio State medical coverage for 2018, you must complete a Health Election Form, and return it to the Office of Human Resources by December 11, 2017.

After January 2, 2018, complete a Your Plan for Health (YP4H) Personal Health and Well-Being Assessment (PHA) with biometrics to earn a premium credit and access to a Health Reimbursement Account (HRA). Learn more about YP4H.

An HRA is an employer-funded account from which you can be reimbursed for eligible heath care expenses. Participants earn credits to their personal HRA as part of the YP4H Incentive Program. An HRA can be used for copayments, coinsurance and deductibles. Learn more about Ohio State’s HRA.
