Complimentary YP4H Biometric Health Screenings are available for winter quarter at 11 stationary locations on campus. The health values you obtain from your biometric health screening—such as blood pressure, blood glucose, cholesterol, height, weight, and Body Mass Index—can be used to complete the Personal Health Assessment (PHA) questionnaire. It's important to complete your PHA every calendar year to continue receiving your PHA Medical Plan Premium Credit. Completing your PHA each year also allows you to participate in the YP4H Faculty & Staff Incentive Program, in which you can earn up to an additional $125. Biometric health screenings are a service provided by The Ohio State University Health Plan Inc.

After your biometric health screening, follow these steps to complete your PHA:
- Register or log in at YourPlanForHealth.com.
- Click Personal Health Assessment under Get Started. Complete the PHA questionnaire and click Submit.
- Review the Personal Health Summary generated by your PHA responses. Feel free to share it with your primary care physician.
Your PHA Medical Plan Premium Credit will automatically begin within
30 days. Within two weeks, you can begin tracking your health-related activities through the YP4H Faculty & Staff Incentive Program.
