Schedule a 20-minute appointment for your free biometric health screening, available to faculty and staff Monday through Friday at a number of locations around campus.
Offered through Your Plan for Health, a biometric health screening measures vital health values (e.g., blood pressure, cholesterol, glucose, weight, and height) that can be used to complete the Personal Health Assessment (PHA). Completing your PHA in 2009 allows you to receive a $15 monthly or $6.92 biweekly discount off your medical premium and qualifies you to participate in the 2009 Faculty and Staff Incentive Program, in which you can earn an additional reward up to $125! Remember, the PHA is a questionnaire and is not the same as the on-campus biometric health screening.
Even if you’re not enrolled in a university medical plan, you can still take your yearly biometric health screening and PHA and be rewarded for participating in the Incentive Program.
Appointments are available almost daily at a number of locations around campus. This year, the screening sites will move around each quarter to better serve employees in different parts of campus. You don’t have to wait for your anniversary date—get your screening at your choice of time, date, and location while there are plenty of appointments to choose from.
