Spouses and same-sex domestic partners can earn $100
Spouses and same-sex domestic partners of Ohio State employees who are enrolled as dependents in a university medical plan can now receive a $100 gift card upon completing their Personal Health Assessment (PHA). To be eligible for the gift card, spouses and same-sex domestic partners must register at YourPlanForHealth.com using their own PIN number provided by WebMD and complete the PHA online.
