Know Your Numbers. Know Your Health.

Did you know that you can schedule your free biometric health screening Monday through Friday at a number of locations around campus? Offered through Your Plan for Health, a biometric health screening measures vital health values (e.g., blood pressure, cholesterol, glucose, weight, and height) that can be used to complete your Personal Health Assessment (PHA).

Completing your PHA in 2009 allows you to become eligible to receive a medical premium reduction up to $180* and participate in the 2009 Faculty and Staff Incentive Program to earn up to $125 for tracking your health and wellness activities. Plus, you’ll get a chance to win an additional $50 though the monthly PHA Award drawings!

Spouses and same-sex domestic partners of Ohio State employees who are enrolled as dependents in a university medical plan also can receive a free biometric health screening at select screening days and sites. Those health numbers can be used to complete their PHAs for a $100 gift card.

For more information or to schedule your biometric health screening at one of the locations and dates below, visit YourPlanForHealth.com and then click on “Register for Screening” under Step 2 on the left.

Remember: As 2009 winds down, available screening appointments will become limited, so make your appointment today!

*Note that as the calendar year progresses, the $180 medical premium reduction becomes pro-rated with each passing month.
