$180 Not Enough? Want $50 More?
With the holiday season approaching, what would you do with an extra $50 in your pocket? The Office of Human Resources will draw 10 lucky winners from faculty and staff members who complete their 2009 PHA. So, in addition to receiving an annual medical plan premium reduction up to $180*, you’ll also be entered into a drawing for an additional $50 cash card.
Monthly drawings will be held in October, November, and December. If you haven’t taken your PHA yet, visit YourPlanForHealth.com today. When completing your PHA, be sure to consent to releasing your PHA completion date (the first of two consent forms). Please note that you won’t be releasing your personal health information, just the date on which you completed your PHA.
* Note that as the calendar year progresses, the $180 medical premium reduction becomes pro-rated with each passing month.
