Your plan for health logoFeel healthier, be more fit, reach your wellness goals. There are many reasons to join the Faculty and Staff Incentive Program, and now payouts will be made quarterly when you earn the maximum points—providing a more immediate reward as an incentive to get started. For example, if you track and earn the maximum 125 Incentive Program points by June 30, you will receive your payout of $125 (less applicable payroll taxes) in July. Acquiring the maximum 125 points by September 30 will earn you an October payout. If you earn less than 125 points during the year, you’ll receive your payout in January. (See chart below for the payout schedule.)

Through the Incentive Program, you can earn points for healthy activities such as being tobacco-free, getting a vision exam, or receiving a flu shot or mist. Each point is worth one dollar (1 point=$1). For example, if an activity is worth 15 points, you’ll earn $15 for completing it.

Joining the Incentive Program is easy. As a benefit-eligible faculty or staff member who has completed the Personal Health Assessment (PHA), you can access your points-tracking profile by visiting yourplanforhealth.com. If you’re a first-time participant, go to the “First Time Here” box to create a personal profile using your employee ID number. Creating a profile is required before any incentive points can be tracked. If you created a points-tracking profile for the 2007 Incentive Program, you can access your existing account by entering a username and password in the “Already have a Profile?” box. Points are tracked confidentially through Matria Healthcare, a third-party provider of health and wellness programs.

Incentive Program Fast Facts

  • Earn up to 125 points ($125) by participating in health-related activities by December 31, 2008.
  • Points will be awarded based on participation, not on outcomes or individual health status. The amount you earn will be paid directly to you through payroll (less payroll taxes), whether it is the $125 maximum or a lesser amount for fewer earned points.
  • Benefit-eligible faculty and staff: Medical plan enrollment is not required.
Affiliated groups: Medical plan enrollment is required.
Graduate Associates (GAs): Enrollment in a university faculty/staff medical plan is required.
  • The PHA is no longer a points-eligible activity for the Incentive Program since taking the PHA entitles you to a medical premium deduction of $120 annually. However, completing the PHA is required for Incentive Program participation. If you don’t complete the PHA, you will not be eligible to log other health-related activities to earn points.
  • PHAs taken in 2007 qualify, however, be sure to take the PHA again this year (and annually) to continue qualifying for future programs including premium discounts.
