Start Earning Points for the 2009 Incentive Program

Your Plan for Health’s 2009 Faculty and Staff Incentive Program is now underway, so start tracking your points for participating in activities, such as exercising, getting a physical exam, and much more. Being healthy offers rewards, and the points you earn equal cash!

New for 2009

Have you visited the new Your Plan for Health web site yet? Simply registering for the site will earn you points for the Incentive Program. Other activities that you can earn points for have been added and include signing up to receive health-related e-mails and enrolling in various online lifestyle centers offered through yourplanforhealth.com. So log in to the new site to start receiving points and customized information to help improve your health.

Also new this year, 125 points will equal $125. For every one incentive point you earn, you’ll receive $1 in payout after applicable taxes through payroll.

Tracking Your Points

Beginning January 1, all benefit-eligible faculty and staff who have completed their Personal Health Assessment (PHA) can access their points tracking profile for the Incentive Program by visiting yourplanforhealth.com and clicking on the “Rewards” link. The points-tracking web site is confidentially maintained by WebMD, a third-party vendor.

Eligibility

  • Completing the PHA is required to participate in the 2009 Incentive Program.
  • Benefit-eligible faculty and staff: medical plan enrollment is not required to participate in the Incentive Program.
  • Affiliated group members: medical plan enrollment is required to participate in the Incentive Program.
  • Participants must meet eligibility requirements when activities are completed to receive payout.
  • Participants must still be employed by Ohio State at the time of incentive payout.
  • Dependents of faculty and staff are not eligible for the 2009 Incentive Program unless they also work for the university and are benefit-eligible.

2009 Your Plan for Health Enhancements

Save more. Complete your PHA to save $15 monthly, or $6.92 biweekly, on medical premiums for 2009. The PHA must be completed annually to maintain the reduction for future medical premiums and to participate in the Incentive Program.

It’s a family affair. Spouses and same-sex domestic partners will now receive a $100 gift card for completing the PHA. Visit yourplanforhealth.com to complete the PHA and for more details about this incentive. Note: Spouses and same-sex domestic partners must register for the site and create an account to be eligible for the gift card.
