Health Advisors Help You Achieve Wellness Goals

The New Year is here and with it comes brand new resolutions to be healthier. While 70 percent of people commit to improving their health in the New Year, only 25 percent are successful. That statistic may seem daunting, but the truth is that many of us just need a little help to change our unhealthy behaviors. A CareAllies Health Advisor, offered though Your Plan for Health, can provide you with the personalized, one-on-one support you need to help overcome the challenge of creating a healthier lifestyle and developing a long-term plan for success!
Formerly known as the Health Coach program, the enhanced Health Advisor program offers many of the same resources, plus a few new ones. The program provides you with several key services, including:
- Wellness and Risk-Based Outreach: Do you have a health concern, such as high cholesterol? Do you need to be more physically active? A Health Advisor will call you to discuss behavior changes that can improve your health concern and customize a wellness program for you.
- Education and Referral: A Health Advisor will refer you to programs that address your specific needs, such as weight loss or smoking cessation programs. Through one-on-one sessions, Health Advisors will also offer educational support and prepare you for needed behavior changes.
- Preventive Care: A Health Advisor will discuss potential gaps in your recommended preventive care, such as cholesterol tests and preventive exams.
- Behavioral Coaching: A Health Advisor provides support as you make behavior changes and checks progress against personalized action plans. Behavioral coaching targets the following health concerns:
- Hypertension
- Preventive care
- High cholesterol
- Exercise
- Pre-diabetes
- Nutrition
Your Health Advisor Will:
- Provide information and educational materials about disease prevention, condition management, and lifestyle modifications to help you understand your health care needs.
- Assist you in setting and achieving your goals for specific health conditions.
- Help you understand how to work with your doctors to choose the right treatment.
- Work with you to reduce risk factors by referring resources, such as:
- Online health information
- Lifestyle management programs for stress reduction, tobacco cessation, or weight loss
The Health Advisor program features unlimited toll-free calls, allowing you to speak with an advisor about any health-related question. The program is confidential, voluntary, and available to benefit-eligible faculty and staff and their enrolled dependents who have completed a Personal Health Assessment (PHA).
How Can I Join?
If a review of your PHA indicates that you have risk factors that need to be addressed, including high cholesterol or high blood pressure, a Health Advisor will call you. Health Advisors offer personalized and confidential support, help identify resources, and develop strategies for improving your health.
