Spouses and Same-Sex Domestic Partners Can Earn $100 with PHA
Spouses and same-sex domestic partners of Ohio State employees who are enrolled as dependents in a university medical plan can now receive a $100 gift card upon completing their Personal Health Assessment (PHA). Gift cards may be selected from among the nation’s leading retailers, restaurants, travel and entertainment companies, and e-tailers.
To be eligible for the gift card, spouses and same-sex domestic partners must register at yourplanforhealth.com using their own PIN number provided by WebMD, and complete the PHA online.
How can spouses and same-sex partners obtain their biometric values?
Biometric values requested in the questionnaire can be obtained from the adult dependent’s physician from an annual physical within the last 12 months.
However, beginning this summer, select campus biometric health screening events will be open to spouses and same-sex domestic partners, so they will have the option to obtain those health values free of charge, just like faculty and staff do now. Schedule appointments at yourplanforhealth.com and click on “Schedule Your Screening” under Step 2 on the left. Things to remember
- Spouses and same-sex domestic partners must register with their own PIN at yourplanforhealth.com. PHAs completed by a spouse or same-sex domestic partner under the employee’s profile will not be eligible for the $100 gift card. New PINs can be requested at yourplanforhealth.com and will be mailed to the home address on file with the university.
- Gift cards will not automatically be mailed to the spouse or same-sex domestic partner. Individuals must select the gift card of their choice and complete the order process by clicking on the “Incentives” page at yourplanforhealth.com.
- Gift cards are considered taxable income and will be included in the employee’s 2009 W-2.
