Didn’t Complete Your PHA in 2008?
It’s Not Too Late to Reduce Your Medical Plan Premiums!
Even if you didn’t complete your Personal Health Assessment (PHA) in 2008, you can still reap the benefits! Take your PHA now to reduce your medical premiums by $15 a month or $6.92 biweekly for the remainder of this year, and lock in your premium reduction for next year! For example, when you take the PHA in 2009, you’re automatically entitled to the premium reduction for 2010. Take it again during 2010 to continue your eligibility for reduced premiums offered in the future.
As an extra bonus, did you know that you become eligible to earn an additional $125 through the Faculty and Staff Incentive Program after you complete your PHA? You’ll be able to start earning points for health-related activities such as exercising or getting a flu shot or your annual physical. The points add up quickly, and before you know it, you’ll have earned 125 points which equals $125! What are you waiting for?
Remember, you can schedule your biometric health screening and take your PHA at yourplanforhealth.com. You can also request a hard copy version of the PHA if you’re not able to complete it online. Contact the Office of Human Resources Customer Service at service@hr.osu.edu, (614) 292-1050, or 1-800-678-6010 to request a hard copy.
