New Incentive for Spouses and Same-Sex Domestic Partners
Spouses and same-sex domestic partners of university employees enrolled in a university medical plan can now receive a $100 incentive upon completing their Personal Health Assessment (PHA). After completing their PHA, spouses and same-sex domestic partners can select a gift card valued at $100 from the nation’s leading retailers, restaurants, travel and entertainment companies, and e-tailers.
To be eligible for the gift card, spouses and same-sex domestic partners must use their personal PIN number to register at yourplanforhealth.com to complete their PHA.
