Prescription Drug Plan

Prescription drug benefits are included under your elected medical plan coverage. Use the links below to find out more about changes to the prescription benefits.

Value Based Prescription Drug Plan (VBD)

Faculty, staff, and their dependents actively participating in the Care Coordination program for management of asthma, chronic obstructive pulmonary disease (COPD), diabetes, and/or heart disease (coronary artery disease or congestive heart failure) are eligible for the VBD. The copay for generic drugs taken for the chronic condition(s) will be waived and the member cost share for formulary brand-name drugs taken for the chronic condition(s) will be reduced by 50 percent for eligible members. If a member chooses not to actively participate in the Care Coordination program, he/she will not be eligible for the VBD.

Beginning 1/1/12 all VBD participants will be expected to use Express Scripts Home Delivery for all of their maintenance medications in order to receive the VBD discount. This includes medications not related to the condition eligible for Care Coordination. More information on the Home Delivery process will be sent to you if you qualify and participate in the Care Coordination Program. For more information contact The OSU Health Plan at 1-800-678-6269.

Dental Plan

Open Enrollment is your opportunity to make changes to your elected dental plan coverage. There are no plan design changes to the university's dental plan. However, effective January 1, 2012 those with employee only dental coverage will have a premium contribution for this plan. Use the links below to review dental plan information for 2012.

Vision Plan

Open Enrollment is your opportunity to make changes to your elected vision plan coverage. There are no plan design changes to the university vision plan. However, effective January 1, 2012 those with employee only vision coverage will have a premium contribution for this plan. Flexible Spending Accounts (FSA)

Flexible Spending Accounts (FSA) allow you to contribute - through payroll deduction on a pre-tax basis - to a spending account which can be used for reimbursement of eligible health and dependent care expenses, as outlined by the IRS.

If you are a current participant in an FSA, you must re-enroll each plan year. Use the links below to review FSA program information for the 2012 plan year.

Short-Term Disability (STD) Plan

Open Enrollment is the only opportunity to cancel existing STD coverage. There is no Open Enrollment for STD. You may apply at any time by completing an Evidence of Insurability Form. Approval of effective date is subject to the STD vendor.

Life Insurance Plans

  • Ohio State has contracted with Minnesota Life as our life insurance vendor effective January 1, 2012.
  • You MUST update your GTLI beneficiary information through eBenefits.
  • During this one-time enrollment period you may enroll or increase your DGTLI and or VGTLI via eBenefits. If no changes are desired, your current coverage automatically transfers to Minnesota Life.
  • Use the online life insurance calculator to estimate how much coverage you will need.
