Medical Plans
Do you want to make any changes to your medical coverage for the 2012 plan year (January 1 – December 31, 2012)?
Yes?
Review the medical plan changes and use eBenefits to make your elections. If you were enrolled in a medical plan in 2011, but do not wish to continue coverage in 2012, you may drop coverage via eBenefits. All medical elections must be submitted by December 2.
No?
If you want to continue your current plan, do nothing further. However, if you are enrolled in PCC you must re-enroll each year. If you are not in a plan and do not want to enroll, do nothing further. Contact OSU Health Plan at (614) 292-4700 or (800) 678-6269 to begin the application process.

Use eBenefits to remove any ineligible dependents from your coverage and change your coverage level, if applicable, for the 2012 plan year. All dependent coverage changes must be submitted by December 2.
Dental, Vision Plans
Do you want to make any changes to your dental or vision coverage for the 2012 plan year (January 1 – December 31, 2012)?
Yes?
Review the dental and vision plans and premiums and use eBenefits to make your elections. If you were enrolled in a dental or vision plan in 2011, but do not wish to continue coverage in 2012, you may drop coverage via eBenefits. All dental and vision elections must be submitted by December 2.

If you want to continue your current plan, do nothing further. If you are not in a plan and do not want to enroll, do nothing further.

Short-Term Disability (STD)
Do you want cancel Short-Term Disability (STD) coverage?
Yes?
To cancel your STD election for the 2012 plan year (January 1 – December 31, 2012) you may use eBenefits. All Short-Term Disability cancellations must be submitted by December 2.
NOTE: You may apply to enroll in the Short-Term Disability (STD) program at any time by completing an election form and Evidence of Insurability form. Approval is subject to the STD carrier.

Group Term Life Insurance
Your university-provided GTLI coverage is administered by Minnesota Life effective January 1, 2012. You must use eBenefits to designate your beneficiary.
Dependent Group Term Life Insurance
You may enroll or increase coverage in the Dependent Group Term Life Insurance (DGTLI) program during this one-time enrollment period.
Note: If you are currently enrolled in DGTLI coverage and do not wish to make any changes, your coverage will automatically transfer to Minnesota Life and your monthly premium will decrease effective January 1, 2012.
