Have you taken the YP4H Personal Health Assessment (PHA)?
Complete the PHA and receive a PHA Medical Plan Premium Credit up to $360 annually ($30/monthly pay; $13.85/biweekly pay). To complete the PHA, log in to YourPlanForHealth.com and click Personal Health Assessment under Get Started.

About Open Enrollment

About Open Enrollment
During Open Enrollment, eligible Ohio State faculty and staff have the opportunity to review, renew, and make changes to select benefit plans. You may:

  • Enroll in, make changes, or cancel Health Plan benefit elections (medical, dental, vision)
  • Add or drop dependents, based on Health Plan eligibility requirements
  • Enroll in Flexible Spending Accounts (FSA) – continued participation requires re-enrollment each plan year
  • Designate Life Insurance beneficiaries
  • Enroll in or increase Dependent Group Term Life Insurance (DGTLI) and/or Voluntary Group Term Life Insurance (VGTLI) coverage
  • Cancel existing Short-Term Disability (STD) coverage

The university requires all health plan members to certify eligibility for any dependent(s) they are covering on the plan(s).  Even if you are making no other changes, you must designate your life insurance beneficiaries, and be sure to review any dependents who are currently enrolled under your health care coverage, in conjunction with the plans' eligibility requirements (for general information on dependent eligibility, click here.)

This Open Enrollment period will not include an enrollment window for Short-Term Disability coverage. For information about enrolling in or making changes to this benefit program, visit the Benefits Home Page.

Important Dates

Be sure to note the following important dates :

November 16, 2012 The beginning of Open Enrollment for the 2012 plan year. Submit your benefit elections electronically through eBenefits or enroll through paper enrollment forms.
November 14 – 29, 2012 The dates when Open Enrollment informational forums will be held on the Columbus and Regional Campuses.
December 2, 2012 The deadline to submit your benefit elections electronically through eBenefits or enroll through paper enrollment forms.
November 16 – December 31, 2012 The period when you can review your 2012 benefit confirmation statement.
January 1, 2012 The date that benefit plan elections made during this Open Enrollment period become effective.
January 1 – December 31, 2012 The plan year during which benefit plan elections made during this Open Enrollment period will be effective. Also the period when Your Plan for Health initiatives will begin.
Autumn 2012 The next Open Enrollment that will take place for benefit elections, which will be effective during the 2013 plan year (January 1 – December 31, 2013).
