Open Enrollment for 2010

Benefits Confirmation Statements are available at https://hr.osu.edu/secure/cs. The 2010 Benefits Confirmation Statements reflect enrollment status in university sponsored medical, dental, vision, flexible spending accounts (FSA), and short-term disability insurance plans that will be in effect from Jan. 1, 2010 until Dec. 31, 2010. Confirmation statements will be accessible online from Nov. 16 – 27.

It is very important to review your confirmation statement and verify that all benefit elections are accurate. In addition, please review all covered dependents to ensure they meet university eligibility guidelines. The benefit coverage that is reflected on the confirmation statement web site will be in effect for the 2010 plan year unless you make changes during the current confirmation statement period (Nov. 16 – 27). This is the only opportunity, outside of a qualifying event, for you to change your benefit elections prior to January 2011.

Benefit elections, as recorded by the Office of Human Resources as of Nov. 14, 2009, are reflected on the confirmation statement.

Note: Due to plan eligibility guidelines, dependent children who reached or will reach the age of 23 between Oct. 28 and Dec. 31, 2009 do not appear on the confirmation statement. A COBRA packet will be sent to employees of dependent children who reach their 23rd birthday during this timeframe.

To view your confirmation statement online please follow the instructions below:

•  Go to the Benefits Confirmation Statement web site at https://hr.osu.edu/secure/cs

•  Login using your valid OSU Internet User ID (name.#) AND the password you have designated for that account (this is NOT the network password you use to log on to your computer).

•  If you have never activated your OSU Internet account through OIT, you may do so at the following site: https://acctmgt.service.ohio-state.edu/Password.html  or at http://8help.osu.edu. Click on “Activate your OSU Internet Username or change your password.”

•  If you have previously activated your account but do not remember your OSU Internet Username or password, please contact OIT at (614) 688-HELP (4357) for assistance.

Once you have logged into the web site, please carefully review and verify that all benefit elections are accurate. If no corrections are needed, print the statement and keep it for your records. If the benefit elections are inaccurate, you may make changes using one of the options below:

Online Changes:

  • Click on the Open Enrollment Online link located on the Benefits Confirmation Statement web site.
  • Make your desired changes.
  • Certify your changes on the Review and Submit page and submit your changes for processing.

Manual Changes:

  • Print the confirmation statement and make corrections directly on it.
  • Sign and date the statement.
  • The confirmation statement with your corrections must be received by November 30, 2009. You can fax your statement to (614) 292-7813 or mail it to the Office of Human Resources Benefits Services, 1590 North High Street, Suite 300, Columbus, OH 43201-2190.

The Office of Human Resources Customer Service Center can print confirmation statements for faculty and staff who do not have Internet access. Please contact the Customer Service Center at (614) 292-1050, 1-800-678-6010, TDD (614) 688-3730, or by e-mail at service@hr.osu.edu, to request a printed copy of your confirmation statement or if you have questions regarding open enrollment.

Review Your Dependents During Open Enrollment

Beginning in January, the university will require ALL health plan members to certify eligibility for any dependent(s) they are covering on the plan(s).  Even if you are making no other changes, be sure to review any dependents who are currently enrolled under your health care coverage, in conjunction with the plans' eligibility requirements (for more information on dependent eligibility, click here).

Your Plan for HealthThe Your Plan for Health web site, powered by WebMD, provides faculty and staff with a one-stop shop to schedule a biometric screening, complete the Personal Health Assessment (PHA), and track Faculty and Staff Incentive Program points.
