Office of Human Resources Life Events

Unfortunately, you have been injured at work. All employees who sustain an incident, injury, or occupational disease while at work must follow these procedures. See the Serious Illness page if you are off work due to a serious illness.

Reporting the Injury
Immediately inform your supervisor or the person in charge of your area when an accident occurs.

Getting Medical Attention
Seek treatment at one of the following locations:

Employee Health Services, Suite 2A University Hospitals Clinic, 456 W. 10th Ave., 7:30 a.m. – 4:00 p.m.

MedOhio Occupational Health Centers (appointment necessary):

West - 86 North Wilson Road
8 a.m. – 6 p.m.
614-293-3500

OSU East - 1492 East Broad Street
8 a.m. – 6 p.m.
614-257-3559

If these locations are closed, go to University Hospitals Emergency Department. If you are outside Franklin County, go to your physician, an emergency care center in your area or the hospital emergency department

Required Documentation and Notification
Complete an Employee Accident Report form (also available from your supervisor, OSHA coordinator or human resource contact). Notify your supervisor if you will be off work, complete an Application for Leave form (including proper medical documentation) and follow your department's medical leave policies. Contact Integrated Disability for assistance with your Workers' Compensation claim.

Filing for Workers' Compensation or Disability
Contact Integrated Disability for help in filing all of the paperwork needed to obtain disability or workers' compensation benefits. The sooner you file, the sooner benefits will be available to you.

Benefits/Compensation While You Are Disabled and Unable to Work
The university pays for your Long-Term Disability (LTD) coverage, which compensates you beginning 90 days after the injury/illness (with an approved claim). You may also have chosen to purchase Select-Term Disability (STD) coverage, which begins after 30 calendar days and is payable up to 2 months (with an approved claim). Returning to Work
Integrated Disability has a Transitional Work Program that assists you in returning to work if you have medical restrictions following an injury/illness.

Social Security
Contact the Social Security Administration for information on disability benefits that may be available to you if you have worked in social security-eligible positions.
