Are my dependents, spouse, or partner eligible for this benefit? This benefit is available to eligible spouses, same-sex domestic partners, sponsored dependents and dependents. [See Dependent Eligibility Guidelines] How/when can I sign up? Eligible employees may enroll in the vision plan upon hire, during Open Enrollment, or if a qualifying status change occurs. To elect this benefit, download and complete a Health Election Form.There is no identification card for vision care. Will this affect my taxes? Some of your out-of-pocket medical expenses may be eligible for reimbursement pre-tax through the Health Care Flexible Spending Account. When can I change my benefit elections? Eligible employees may change their vision plan elections upon hire into a benefit-eligible position, during Open Enrollment, or if a qualifying status change occurs. Will I receive a card? No - there is no vision plan card. Tell your provider that you have the Vision Service Plan. Visit the VSP Web site at vsp.com/member/htmls/member_home.jsp. Register for an Account and Log In. Under “Benefit Resources” select “Member Reference Card” Have there been any recent changes to this benefit? No. Who do I contact regarding claims? You can visit the VSP web site to verify eligibility for services and view your claim history. What would cause this benefit coverage to end? This benefit would end if the employee transfers to a non-benefit-eligible position, leaves university employment, or if a dependent becomes ineligible.
