Dental Plan Do you have a toothache? Does your child need braces? Then Ohio State's dental care plan may be just what you need. The plan provides coverage for many of the dental services you and your dependents will need.

Are my dependents, spouse, or partner eligible for this benefit? This benefit is available to your eligible spouse, same-sex domestic partner, sponsored dependents and dependents. [See Dependent Eligibility Guidelines] How/when can I sign up? Eligible employees may enroll in the dental plan upon hire, during Open Enrollment, or if a qualifying status change occurs. To elect this benefit, download and complete a Health Election Form. Will I receive a dental plan card? No - there is no dental plan ID card. Tell your provider that you have Delta Dental. You may print a card by visiting the Delta Dental of Ohio Web site at consumertoolkit.com and choose the “Print ID Card” option. Will this affect my taxes? Some of your out-of-pocket dental expenses may be eligible for reimbursement pre-tax through the Health Care Flexible Spending Account. When can I change my benefit elections? Eligible employees may change their dental plan elections upon hire into a benefit-eligible position, during Open Enrollment, or if a qualifying status change occurs. What would cause this benefit coverage to end? This benefit would end if the employee transfers to a non benefit eligible position, leaves university employment, or if a dependent becomes ineligible.
