- Dependent Eligibility Certification
The university is requiring faculty and staff to certify eligibility for any dependent(s) they are covering on the health plan(s). The university has contracted with ACS HR Solutions to perform the eligibility certification. Read more. - YP4H Incentive Payout Delay – Monthly Faculty and Staff
Please note there will be a delay in the processing of the 2009 Your Plan For Health Incentive Program payout for monthly paid faculty and staff. This payout includes those individuals who earned the full 125 points from Oct. 1 – Dec. 31, 2009, and all other point values earned throughout 2009. As a result of this delay monthly paid faculty and staff that were eligible for the payout will see the Incentive Payout added to their February check. An e-mail will be sent to those affected faculty and staff members. Please note the biweekly faculty and staff payout will be processed for the January 29, 2010 pay period. - Hit the Road with the Buckeyes 2009 Prize Winners Announced!

There’s yet another reason to complete your Personal Health Assessment (PHA)! Starting in February, the Office of Human Resources will draw 10 lucky winners every month from those faculty and staff members who complete their PHA during 2010. Each winner will be awarded a $50 debit card. That’s in addition to receiving the medical plan premium reduction — $20 a month or $9.23 biweekly — for completing the PHA.
